ΕΡΩΤΗΜΑ - ΑΙΤΗΜΑ ΑΠΟ ΤΗΝ Dr Mestheneos

Αγαπητοί συνάδελφοι,

παραθέτω εδώ απόσπασμα από το e-mail της Dr Mestheneos η οποία ζητάει από εμάς πληροφόρηση στην αναζήτηση της για υπηρεσίες ή προγράμματα που σχετίζονται με χρόνια νοσήματα , παθήσεις κλπ. Παρακαλώ , όποιος γνωρίζει πρόγραμμα που εμπίπτουν στα πιο κάτω κριτήρια, να μου το γνωστοποιήσει ούριο στη σχολή ή να αφήσει σχόλιο εδώ, προκειμένου με τη σειρά μου να προωθήσω τα στοιχεία επικοινωνίας του στην  Dr Mestheneos.

If the students know of projects/ programs which are concerned with the management of multiple chronic diseases e.g. diabetes and heart disease, alzheimer and CV, HIV and diabetes- etc. At this moment I just need their names and contact details - so I can phone them and find out if it qualifies. - I send the basic criteria below…

Inclusion criteria for programmes/initiatives

We are searching for programmes (or approaches, initiatives, projects, etc.) that meet the following criteria:

• focus on providing care for adult people with multimorbidity (or contain specific elements for this target group), and
• Should be aimed at a patient target group consisting of people aged 18 and older, with two or more medically (i.e. somatic, psychiatric) diagnosed chronic (not fully curable) or long lasting (at least six months) diseases, of which at least one has a (primarily) somatic/physical nature, and
• involve one or more medical service(s), and
• involve cooperation between at least two services (these services may be part of the same organization, for example services within a hospital, or may be part of different organizations, for example between medical care and social care), and
• have some formal status/formalized cooperation (any form), and
• are evaluable in some way, and
• are currently running or finished less than 24 months ago or starts within the next 12 months.
